Cause of Paediatric Autoimmune Neuropsychiatric Disorder

Various studies show that group A streptococcal bacterial infection causes an immune response that results in different neuropsychiatric symptoms associated with PANDAS. Group A streptococcal bacteria is the same bacteria that causes strep throat. It is considered that strep bacteria tend to hide from the body’s immune system and survive in the human host for a long duration. It does so by putting molecules on its cell wall that mimics the molecules found on a child’s skin, brain tissues, joints, and heart. Due to the similarity of the cell membrane of bacteria and the locations mentioned above, the immune system is not able to detect the bacteria for a long time. When our immune cells recognize these foreign bacteria, they start producing antibodies to fight against them. Symptoms of Paediatric Autoimmune Neuropsychiatric Disorder

There are many symptoms that are noted in cases of PANDAS. The major symptoms of PANDAS include OCD (obsessive-compulsive disorder) and tics. The OCD symptoms include frequent hand washing, ritualistic behavior and many more.
Tics refer to semi-voluntary repetitive movements or sound production that involves distinct muscle groups. The tics like throat clearing, eye twitching, and head shrugging are frequently seen in PANDAS. In addition to these ADHD symptoms may arise. Other attending symptoms include anxiety, refusal to go to school, separation anxiety (child wants to cling to the parents or caregiver). Diagnosing PANDAS

There are no laboratory tests available that can diagnose a given case of PANDAS. PANDAS is suspected in a child based on the sudden onset of neuropsychiatric symptoms following a streptococcal infection. Following are the guidelines that help in diagnosing PANDAS:
1. Presence of OCD (obsessive-compulsive disorder) and/or multiple tics.
2. Very sudden and abrupt onset/ worsening of symptoms.
3. The onset of the symptoms between age 3yrs to pubertal age group.
4. Association with group A streptococcal infection.
5. Neurological abnormalities (physical hyperactivity or unusual jerky movements).

PANDAS and Autism

PANDAS and Autism are two different disorders. However, these two conditions can coexist in some children. But there is no clear evidence regarding the percentage of occurrence of PANDAS in children with autism as compared to those who do not have autism. It is also difficult to reach a diagnosis for PANDAS in a child with autism. This is because of the symptom similarity that these common disorders share with each other. The major symptoms shared include repetitive behaviors, sensitivities (towards touch, light, sound). One differentiating point to be noticed here is that the symptoms in PANDAS develop abruptly and all of sudden as compared to autism.
